Childbirth with Dignity Foundation
Childbirth with Dignity Foundation monitors perinatal care in Poland. In order to increase the scale of monitoring, the Foundation decided to create an online tool that would be another method (along with the work of field interviewers) for collecting information from women who have given birth.

The challenge was to design an on-line tool that would automate the process of survey data collection, analysis and presentation of data.
The main requirements were to support multiple question types, support conditional question sections, support analysis and presentation of large volumes of data.
We built a web-based application to enable flexible survey tool design, user anonymization system, and data analysis and presentation modules.
After analyzing additional requirements to automate the tedious work of manually building reports for hospitals, we built a tool that generates reports automatically.
The Childbirth with Dignity Foundation was able to create a survey with 150 questions of various types, including conditional questions, using the new tool. The survey works at ankieta.rodzicpoludzku.pl, the survey data is used to automatically generate reports for hospitals and is used to create a database of hospitals in the gdzierodzic.info/szpitale server.
As of 2018, users have already answered 8 228 933 questions. As of 2021, 19 629 surveys have been completed, and nearly 100 000 surveys have been completed since the survey began. 65% of women complete the survey they started.
